State-funded providers are reminded that Partners will begin transitioning current Medicaid members with intellectual/developmental disabilities (I/DD) that meet Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF-IID) level of care from Supported Living and Supervised Living state residential services to Long-Term Community Supports (LTCS Level 2: semi-independent living and LTCS Level 5: 4-6 bed group homes) beginning Oct. 1, 2021. LTCS is a Medicaid In-Lieu of Service definition and may be viewed at https://providers.partnersbhm.org/service-definitions/.
Partners looks forward to working with providers to make this a seamless transition, without service disruption, for Medicaid members that meet the ICF-IID level of care. Members that do not have Medicaid or do not meet the ICF-IID level of care criteria will continue to receive State funded Day or Residential Services per Partners Benefit Plan.
To ensure a smooth transition, Partners will hold two virtual Provider Education Sessions regarding LTCS. Please plan to attend one of these required sessions as well as review the LTCS eligibility criteria on the Partners’ Provider Knowledge Base.
