I/DD Patient Support Overview

Individuals living with an intellectual and/or developmental disability (I/DD) may require additional supports to maintain their health, increase independence and improve quality of life. This page provides medical professionals with curated materials and referral resources to support patients with I/DD and their caregivers.

Many patients and families are unaware of available home- and community-based services. Providers play an important role in identifying needs, initiating referrals and connecting families to appropriate supports. The resources below are designed to assist with care coordination, service navigation and health promotion.
