Members receiving State funded, B3, or ICF-IID services must have a valid NC-SNAP assessment in their medical record unless they have a SIS assessment. NC-SNAPs are valid for 1 year.
Any NC-SNAP documents that have not been submitted by Fax or Email to Partners must be submitted by the end of business, September 30. All pages including the cover sheet are required. Changes in status may be submitted on the cover sheet.
For Rutherford County providers – Any NC-SNAP documents that have not been uploaded to AlphaMCS must be uploaded to AlphaMCS and Faxed or Emailed to Partners by the end of business, September 30. All pages including the cover sheet are required.
